4AT evidence base

The research base: diagnostic accuracy, reviews, guidelines and routine practice.

The 4AT has the most formal diagnostic accuracy data of any delirium tool, around double that of the Confusion Assessment Method (CAM), or Nu-DESC.

In view of the strong evidence base underlying the 4AT, including a head to head diagnostic test accuracy RCT comparing 4AT with the CAM, in 2023 the UK’s National Institute for Health and Care Excellence (NICE) updated its official delirium guideline, replacing the CAM with the 4AT.
